Bright Pathways Housing Assistance Program
Bright Pathways provides need-based rental assistance to families caring for children with medically complex disabilities. To ensure fair and responsible distribution of funds, applicants must meet the following eligibility requirements.

Child Eligibility Requirements
To qualify, the child must:

Be under the age of 18
-
Have a documented medically complex disability
-
Be actively enrolled in Medicaid or an approved Medicaid waiver program
-
Be actively receiving home care services OR in the process of being staffed
-
Receive services from a Bright Pathways Grant-Authorized Home Care Provider

Examples of qualifying medical conditions include, but are not limited to:

Cerebral palsy
-
Seizure disorders / Epilepsy
-
Traumatic brain injury (TBI)
-
Hydrocephalus
-
Severe developmental delay
-
Spina bifida
-
Muscular dystrophy
-
Spinal muscular atrophy (SMA)
-
Congenital heart defects
-
Extreme prematurity with ongoing medical needs
-
G-tube or J-tube dependence
-
TPN dependence
-
Severe feeding or gastrointestinal disorders
Medical documentation is required.

Home Care Provider Requirement
Families must be actively receiving services from one of the following Bright Pathways Grant-Authorized Home Care Providers:

Utah – Canyon Home Care
-
Texas – Pediatrics Connect
-
New York – Attending Home Care
-
Georgia – Strength in Care
-
Wyoming – Essential Living Support Inc.
-
Wisconsin – Comfort Personal Care Services
If a family is not yet staffed, enrollment assistance may be available through an approved provider.

Housing Requirements
Applicants must:

Have a valid lease or mortgage
-
Be financially responsible for the residence
-
Reside in the home with the child
-
Have a landlord or mortgage provider willing to accept third-party payment
Payments are made directly to the landlord or mortgage company.

Financial Hardship Requirement
Applicants must demonstrate documented financial hardship, such as:

Loss of employment
-
Reduced work hours due to caregiving responsibilities
-
Medical emergency
-
Hospitalization
-
Eviction notice
-
Increased medical or disability-related expenses
Proof of income and supporting documentation are required.

Who May Apply?
The applicant does not have to be the biological parent.
Eligible applicants include:

Parent
-
Legal guardian
-
Grandparent
-
Primary caregiver
The applicant must reside in the home and be financially responsible for housing.

Assistance Details

Eligible families may receive up to $2,000 per month

Amount awarded depends on documented need and available funding
-
Approval decisions are typically made within 7–14 business days after all required documents are received
-
Funding is limited and subject to availability
